• CONSEF Membership Form

  Please make sure that all information is correct. Spell names as you would want them spelled on a certificate.

  Membership fee is $100.00 which covers registration fees of 10 projects.

  The CONSEF will not accept registrations from schools received after February 12, 2010.

  Each school or building in a district must register as a separate member school.

  In addition to e-mail or postcard notification, current members will be posted on the CONSEF website www.consef.org

• Contributing Member Form

  Thank you for your interest in becoming a Contributing Member of the Concept Science and Engineering Fair (CONSEF). The mission of CONSEF is to increase public awareness in science, math and engineering.

• Student Application Form

  This completed form is required for ALL projects. Type or print all information requested.

• Release and Consent Form

  In accordance that the undersigned shall be participating and attending Concept Schools Science & Engineering

  Fair, and any related activities and events organized by Concept Schools, the undersigned hereby for himself/herself

• Checklist for Adult Sponsor / Safety Assessment Form (1)

  This completed form is required for ALL projects and must be completed prior to experimentation.

• Abstract (1A)

  The abstract should include the following:

  A. Question being addressed

  B. Hypothesis/Problem/Engineering Goals

  C. Description in detail of method or procedures (including chemical concentrations and drug dosages)

  For human research, include survey or questionnaires if used, and critically evaluate the risk. For nonhuman vertebrate animal research, you must briefly discuss POTENTIAL ALTERNATIVES and present a detailed justification of use of nonhuman vertebrate animals.

• Approval Form (1B)

  This completed form is required for ALL projects.

• Registered Research Institutional/Industrial Setting Form (1C)

  This form must be completed by the scientist supervising the student research conducted in a registered research institution (e.g., universities, medical centers, correctional facilities, etc.) or industrial setting.

• Qualified Scientist Form (2)

  Required for research involving animals, controlled substances and pathogens; may be required for rDNA, tissues, and humans. Must be signed prior to the start of student experimentation.

• Designated Supervisor (3)

  Required if the Qualified Scientist is unable to supervise the experiment OR for projects using hazardous materials or devices.

• Human Subjects Form (4)

  Required for all research involving humans. IRB approval required before experimentation.

• Non-Human Vertebrate Animal Form (5)

  Required for all research involving nonhuman vertebrate animals. (SRC approval required before experimentation.)

• Human and Non-Human Vertebrate Animal Tissue Form (6)

  Required for all projects using viable fresh tissue, organs, human or animal parts, including blood, blood products, teeth, primary cell cultures, and body fluids (plant tissue is excluded). (SRC approval required before experimentation.)
